Translating Proposed Criteria into a Clinical Tool for the Diagnosis of Functional Tic-Like Behaviors in Youth: Refining the Criteria and Reliability Findings

Description

The prevalence of patients presenting functional tic-like behaviors (FTLBs) increased significantly during the COVID-19 era. FTLBs represent a phenotype distinct from neurodevelopmental tics; for example, this presentation typically onsets later and does not follow the typical phenomenological course as Tourette Syndrome (TS). The European Society for the Study of TS (ESSTS) proposed clinical criteria to define this presentation. However, FTLBs are not yet included in diagnostic classification systems and there are mixed results on whether certain criteria are necessary for making a diagnosis. Through this study, researchers developed a decision-making tool using the ESSTS criteria and applied this tool to 114 medical encounters of children referred for tics at Lurie Children’s Hospital of Chicago between 2022-2024. Good interrater reliability was established for the tool and moderate reliability for matching the diagnosis made by the neurologist. Data on the most versus least important clinical criteria established through this study will also be presented.
